What is a mandibular advancement device?
A medical device, the mandibular advancement device is only given on medical prescription to treat snoring and respiratory pauses or obstructive sleep apnea. Its purpose is to clear the airways, particularly the airway intersection between the pharynx and the larynx. This dentally anchored device is made of strong and flexible polymer (resin). It includes two semi-rigid aligners attached to each other by an advancement system. It also has interchangeable adjustment rods to control mandibular advancement and a hinge at the back depending on the occlusion of the teeth. The mandibular advancement device is custom-made according to the patient's dental impressions. It is like a mouthguard, but it is less bulky with lower and upper parts that can be moved and fixed together to keep the lower jaw or mandible in a more forward position than normal. The advancement of the mandible to which the tongue is attached allows the airways to open by preventing the tongue from falling to the back of the throat during sleep, which obstructs the airways.
Conditions for fitting the mandibular advancement device
Certain conditions must be met to be able to fit a mandibular advancement device . You must have enough teeth, at least 6 per jaw, for this device to be well retained. You must also have a healthy periodontium, without gum inflammation. You must also not have joint problems, significant prognathism or cardiovascular disease. Wearing this device during sleep is recommended in cases of mild to moderate sleep apnea. If it is severe, continuous positive airway pressure ventilation is required.
